How does Biomet 3i Cam StructSURE CAD CAM work?
From a clinical perspective you can carry on with the same procedures you are familiar with when having your bars and beams restored. To demonstrate the usual procedure there is a workflow chart below.![]()

Why use CAD CAM over the conventional cast bars?
Cast gold bars are made using gold cylinders similar to the ones used for gold custom abutments and this along with the complexity of the cases makes them very expensive. These restorations are a real test of the skill and knowledge of the laboratory producing them due to the minute tolerances involved in producing a large span passive fitting cast gold structure.
These may have fitted well and looked great initially but some studies of cast gold bars 6 years after fit show high levels of plaque adhesion due to micro porosity which was not evident at fit, there is no chance of porosity in milled titanium bars as they are milled from one homogenous block of titanium. Also cast gold bars can contain stresses due to the gold casting process which can cause problems later on, Which is why we always try to use a CAD CAM solution whenever possible.
Other complications of traditional cast units.
This picture shows the potential casting defects that can occur on cast gold cylinders. This cylinder was recieved by the lab from a failed case where the patient had tissue inflamation and an infection. We believe this may have been due to the porosity void on the left as well as gaps between the cast yellow gold and white cold cylinder almost all the way along the junction which showed up after cleaning in an ultrasonic bath. These voids are a haven for plaque and bacteria which could cause some serious problems later on, especially as this section is usually subgingival and in constant contact with the tissue.
We should make it clear that obviously the picture shown is a flawed case which failed due to poor workmanship. Most labs would ensure good homogenous casts to the very best of their ability and never allow this kind of restoration to leave the laboratory but it does show that there is an inconsistency in the quality of cast gold bars which is not present in milled titanium bars.
If we take that example and also add the large volumes of alloy required for some of these frameworks we really are into a world of potential problems. For example the diagram below shows a structure we made in the lab to carry a 12 unit bridge. This needed quite a large framework or we would have had insufficient support and therefore increased risk of composite fracture later on. The pontic section marked is 9mm by 9mm! Casting this and ensuring no porosity would be almost impossible and would be sure to weigh a great deal more than the milled framework which would have cost implications. The milled framework we had made was a stress free homogenous structure which also had a great cost saving on the final restoration.

Thanks to the use of eDrawings software, all proposed Biomet CamStructSURE 3i CAD CAM bars can be viewed in a fully interactive 3D viewer. By using the eViewer (shown below) we can instantly share the images with our surgeons to discuss any issues and ensure that the final bar meets your requirements prior to milling. This allows us to have far more imput into the design as a team and thanks to the eViewer we can inspect the proposed bar form all angles as well as with or without the models or try in visible.
